Community Health Needs Assessment: UC Davis Medical Center (2016)
All nonprofit hospitals must conduct a Community Health Needs Assessment (CHNA) every three years and adopt an implementation strategy to meet the community health needs identified through the CHNA.
The CHNA must define the community served by the hospital, solicit input from broad interests of the community, assess the health needs of the community, prioritize those health needs and identify potential measures and resources available to address the health needs.
This document is the final 2016 report for the UC Davis Medical Center service area, authored by the Valley Vision health team.
