Community Health Improvement Plan: San Joaquin County
Recognizing the need for targeted, coordinated priorities and goals to drive change, more than 100 stakeholders contributed to the development of the Community Health Improvement Plan for San Joaquin County. The CHIP was created to guide collaborative, long-term efforts to address community health needs throughout the county. Knowing that even a well detailed plan will not fully mobilize the actions desired, the Healthier Community Collaborative engaged Valley Vision to help advance priorities, goals, and strategies outlined in the Community Health Improvement Plan (CHIP).
Valley Vision aligned efforts by lifting up and prioritizing common priorities, creating a centralized communication hub to collect and distribute updates and opportunities, facilitating regular stakeholder meetings, defining and monitoring success metrics, researching and sharing funding opportunities, and coordinating collaborative funding proposals.
